Please note that Well Women Physical Therapy is an out-of-network provider and does not contract directly with insurance companies.

 

Unfortunately, because physical therapists are not permitted to “opt out” of Medicare/Medicaid, we are unable to work with these beneficiaries even if they would like to pay directly.​

For all other patients:

• Payment is collected at the time of service.

• Patients may request a detailed superbill that they may submit to their insurance company for possible out-of-network reimbursement.

• Reimbursement is determined by the patient’s individual plan.

• Prices are completely transparent and listed below.
